IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT * USA PATRIOT ACT
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account.
What this means for you: When you open an account, we will ask for you name, address, date of birth, and other information that will allow us to identify you. We may ask to see your driver’s license or other identifying documents.
We require the presentation of at least two forms of government issued ID such as: driver’s license; state ID; passport, birth certificate, Social Security Card, or military ID for some examples. This requirement also applies to joint owners on the account, if any.
You will need to complete a membership application and deposit $5.
Provide the above identity forms (at least 2).
If you are wanting to setup a direct deposit, may we suggest avoiding the first three days of the week of “payroll week” because ProvConnect is unavailable to set up a new direct deposit.
